Office Based Opioid Treatment (OBOT) is an American initiative which helps to make opiate agonist treatment available to individuals who cannot access methadone or levo-alpha-acetyl-methadol (LAAM) treatment.

OBOT is primarily used for individuals struggling with opioid addiction, such as addiction to prescription painkillers or heroin. The treatment is provided by specially trained doctors, known as addiction medicine physicians or addiction psychiatrists, in their office setting. These physicians utilise FDA-approved medications to help you manage your withdrawal symptoms and cravings and ultimately aid in your recovery. The Office-Based Opioid Treatment (OBOT) model involves primary care providers prescribing medication for opioid use disorder (MOUD) in their clinical settings after undergoing specific training and obtaining a waiver. This model integrates MOUD into a patient’s primary care, typically with the primary care provider prescribing medication independently, although support staff like nurses or social workers may assist in coordinating services. In OBOTs, patients can take buprenorphine home and receive naltrexone on site.

What Is the Difference between OTP from OBOT?

Unlike Opioid Treatment Programs (OTPs) that offer comprehensive outpatient treatment including pharmacotherapy for opioid use disorder, Office-Based Opioid Treatment (OBOT) primarily involves practitioners prescribing medications for this disorder in a non-OTP setting. While OBOTs are not required to provide behavioural health services, it is recommended to connect patients with community-based mental health services if such services are not available within the OBOT framework.

One of the commonly prescribed medications in OBOT is buprenorphine. Buprenorphine is known as a partial opioid agonist, which means it binds to the same receptors in the brain as opioids but produces a diminished effect. It helps to reduce cravings and withdrawal symptoms, allowing you to focus on your recovery without the intense discomfort often associated with quitting opioids cold turkey.

OBOT also typically includes counselling and behavioural therapy as part of the treatment plan.
